Access Independent service referral process
The referral process varies according to the type of service being provided:
Clearing waiting lists
- Customers may request a quotation prior to placing an order
- Once the funding has been agreed, a service level agreement is drafted by Access Independent for approval
- When the contract is implemented, the customer forwards cases in batches to Access Independent for allocation to their assessors
Mobility assessments
- The applicant contacts the relevant local authority, which forwards the application to Access Independent as part of a batch if the applicant does not meet the automatic criteria
- Access Independent books a clinic appointment with the applicant, unless otherwise agreed at the beginning of the contract
One-off assessments
- Potential customers should request a quotation
- Once price is agreed, the details are supplied and Access Independent drafts a 'work agreement" for the assessor
- For established customers, there is no need to ask for a quotation each time. A referral form can be completed detailing the client's name, address, present situation and reason for referral
- The case is then allocated to an OT assessor
